❶ 醫院繼續教育管理制度
第一章 總則
第一條 為實施「科教興國」戰略，適應社會主義衛生事業發展需要，國家對衛生技術人員實行繼續醫學教育制度。依據《教育法》、《執業醫師法》和《全國專業技術人員繼續教育暫行規定》，特製定本規定。
第二條 繼續醫學教育工作應適應醫學科學技術的發展和社會的實際需要，面向現代化，面向世界，面向未來。
第三條 繼續醫學教育是繼畢業後醫學教育之後，以學習新理論、新知識、新技術、新方法為主的一種終生教育。繼續醫學教育的目的是使衛生技術人員在整個職業生涯中，保持高尚的職業道德，不斷提高專業工作能力和業務水平，提高服務質量，以適應醫學科學技術和衛生事業的發展。
第四條 繼續醫學教育的對象是完成畢業後醫學教育培訓或具有中級以上(含中級)專業技術職務從事衛生技術工作的人員。參加繼續醫學教育是衛生技術人員應享有的權利和應履行的義務。
第二章 組織管理
第五條 繼續醫學教育工作實行全行業管理。各級衛生行政部門要打破醫療機構的行政隸屬關系和所有制界限，充分利用各地區的衛生和醫學教育資源，按照專業技術人員繼續教育的總體要求，加強對繼續醫學教育工作的規劃、組織和領導。全國和各省、自治區、直轄市繼續醫學教育委員會是指導、協調和質量監控的組織。
第六條 全國繼續醫學教育委員會在衛生部、人事部的領導下，由衛生部、人事部、解放軍總後衛生部、有關衛生廳(局)、人事廳(局)，高等醫學院校、學術團體和醫療衛生單位的領導及專家組成。委員會下設若干個學科組，聘請有關專家擔任學科組成員。委員會的職能是：
1．研究全國繼續醫學教育的方針、政策，向衛生部、人事部提出建議；
2.研究和提出全國繼續醫學教育的總體規劃和實施計劃；
3．負責擬訂繼續醫學教育項目的評審標准，申報、認可辦法和學分授予辦法等；
4．負責國家級繼續醫學教育項目的評審。評審結果作為衛生部批准和公布的依據；
5．組織選編、出版國家級繼續醫學教育項目的優秀文字教材、音像教材和遠程繼續醫學教育課件；
6．開展遠程教育，推動全國繼續醫學教育廣泛深入地開展；
7．對省級繼續醫學教育委員會、衛生部直屬單位及相關單位的繼續醫學教育工作進行指導、檢查和評估；
8．負責國家級繼續醫學教育基地的評審。評審結果作為行政主管部門批准和公布的依據。
第七條 省級繼續醫學教育委員會在省級衛生行政部門、人事行政部門的領導下，由衛生廳(局)、人事廳(局)和有關單位的領導及專家組成。委員會下設若干個學科組。委員會的職能是：
1．擬訂本省(自治區、直轄市)繼續醫學教育的規劃和計劃；
2．依據繼續醫學教育有關規定，擬訂實施細則；
3．評審省(自治區、直轄市)級繼續醫學教育項目。評審結果作為省級衛生行政部門批准和公布的依據；
4．組織繼續醫學教育文字教材、音像教材和遠程繼續醫學教育課件的編寫、 出版和發行工作，開展遠程教育；
5．對本省(自治區、直轄市)的繼續醫學教育工作進行指導、檢查和評估；
6．評審本省(自治區、直轄市)繼續醫學教育基地。評審結果作為行政主管部門批准和公布的依據。
第八條 地(市)、縣兩級衛生行政部門應加強對繼續醫學教育的領導，負責貫徹落實省級繼續醫學教育的計劃和要求，組織各項活動。
第九條 各單位要為衛生技術人員參加繼續醫學教育提供必要的條件。衛生技術人員要積極主動參加繼續醫學教育活動，並按照繼續醫學教育的有關規定，服從所在單位的安排，接受考核。在學習期間享受國家和本單位規定的工資、保險、福利待遇。在接受繼續醫學教育後，有義務更好地為本單位服務。
第十條 各級衛生、人事行政主管部門應加強對繼續醫學教育工作的領導，並從制度上予以保證。各醫療衛生單位、高等醫學院校和學術團體應將開展繼續醫學教育作為一項重要任務，認真落實。
第三章 內容與形式
第十一條 繼續醫學教育的內容，應以現代醫學科學技術發展中的新理論、新知識、新技術和新方法為重點，注意先進性、針對性和實用性，重視衛生技術人員創造力的開發和創造性思維的培養。根據學科發展和社會需求，開展多種形式的繼續醫學教育活動。
第十二條 在繼續醫學教育活動中要注意加強政治思想、職業道德和醫學倫理學等有關內容的教育，培養高素質的衛生技術人員。
第十三條 繼續醫學教育堅持理論聯系實際，按需施教，講求實效的原則，根據學習對象、學習條件、學習內容等具體情況的不同，採用培訓班、進修班、研修班、學術講座、學術會議、業務考察和有計劃、有組織、有考核的自學等多種方式組織實施。
第十四條 各地區、各單位應根據不同內容和條件，採取靈活多樣的形式和辦法，開展以短期和業余學習為主的繼續醫學教育活動。自學是繼續醫學教育的重要形式之一，應有明確的目標，制定自學計劃，經考核認可授予學分。相應的自學管理辦法由省級衛生行政部門制定。
第十五條 衛生部和省級衛生行政部門定期將認可的繼續醫學教育項目，按學科專業分類提前公布，供各地衛生技術人員選擇參加。
第十六條 經審批認可的繼續醫學教育項目分為國家級和省級。全國繼續醫學教育委員會評審國家級繼續醫學教育項目。此類項目按《國家級繼續醫學教育項目申報、認可試行辦法》辦理。省級繼續醫學教育委員會負責評審省級繼續醫學教育項目，此類項目按各省(自治區、直轄市)制定的省級繼續醫學教育項目申報、認可辦法辦理。
第十七條 接受繼續醫學教育的衛生技術人員應根據本人的實際情況和工作需要，選擇參加與本人專業和崗位工作相關的繼續醫學教育活動。
第十八條 解放軍總後衛生部、高等醫學院校、科研院所根據本系統和單位的具體情況及特點，在做好本單位繼續醫學教育工作的同時，要積極面向社會，開展繼續醫學教育。
第四章 考核、登記、評估
第十九條 要對衛生技術人員參加繼續醫學教育活動進行考核。繼續醫學教育活動主辦單位負責考核，衛生技術人員所在單位負責審核。考核、審核的具體辦法由各省級衛生行政部門會同人事行政部門共同制定。解放軍總後衛生部、衛生部直屬單位考核、審核的具體辦法由各單位制定。
第二十條 繼續醫學教育實行登記制度。繼續醫學教育活動主辦單位應對參加活動的衛生技術人員發放本單位簽章的包括活動名稱、編號、形式、日期、考核結果、學分類別、學分數等內容的登記證或學習證明。各單位應建立繼續醫學教育檔案，對本單位衛生技術人員每年參加各種繼續醫學教育活動和獲得的學分進行登記。
第二十一條 繼續醫學教育實行學分制。繼續醫學教育對象每年都應參加與本專業相關的繼續醫學教育活動，學分數不低於25學分。學分的授予和登記應嚴格執行繼續醫學教育學分授予的有關規定。
第二十二條 衛生技術人員接受繼續醫學教育的基本情況作為年度考核的重要內容。繼續醫學教育合格作為衛生技術人員聘任、技術職務晉升和執業再注冊的必備條件之一。
第二十三條 各單位開展繼續醫學教育工作的情況，應作為對領導幹部政績考核的內容之一。
第二十四條 建立繼續醫學教育的評估制度。全國繼續醫學教育委員會和省級繼續醫學教育委員會定期對開展繼續醫學教育情況進行檢查評估。
第二十五條 對積極開展和踴躍參加繼續醫學教育活動，並且成績顯著的單位和個人，應給予表彰和獎勵。
第五章 經費
第二十六條 繼續醫學教育所需的經費，採取國家、集體、個人等多渠道籌集的辦法解決。各級衛生行政部門應將繼續醫學教育經費列入預算。各衛生單位應保證一定的繼續醫學教育費用，並通過其它途徑籌集資金。繼續醫學教育經費要專款專用。衛生技術人員本人也應承擔一定的費用。
第二十七條 根據原國家教育委員會、國家計劃委員會、財政部《關於頒發義務教育等四個教育收費管理暫行辦法的通知》(教財[1996]101號)的有關規定，舉辦繼續醫學教育活動可收取合理學習費用，但不以盈利為目的。
第六章 附則
第二十八條 護士(助產土)的繼續教育暫依照《繼續護理學教育試行辦法》執行。
第二十九條 省級衛生行政部門會同人事行政部門可參照本規定，結合實際，制定具體的實施細則。
第三十條 本規定自發布之日起施行，1991年衛生部發布的《繼續醫學教育暫行規定》同時廢止。
第三十一條 本規定的解釋權屬衛生部。

Surgery Residency Personal Statement
Surgery was my dream long before it came to be real life in medical school. I had begun to appreciate the enormous scope and depth of the field of medicine while still a very young man and my suspicions have been confirmed. The vastness of what we have still to learn refreshes my spirit and inspires me to never become complacent always struggling to make surgery as efficient and effective as possible. This struggle sustains my sense of dignity and self-worth, making each day a joy to live. I am a 27 year-old Indian man who now lives in New York, with the Indian equivalent of the M.D. Degree. In addition to English and Hindi, Telugu is my mother tongue and my primary language of study until it was changed to English while I was in high school. I preserve the rich flavor of my native India and have overcome many obstacles in my quest to become fully professional in English communication. During in my first year of medical school, I used to accompany my mom who is a pharmacist whenever she went to health camps, to check blood pressure and draw blood from patients, thereby learning a lot from my mom. I also formed a student organization along with my classmates dedicated to concting free blood donation camps, health information camps. We also adopted a small slum area "Nandanavanam" and operated weekly health and immunization camps, along with AIDS awareness, and Malaria eradication programs. I had the privilege of helping several of my senior colleagues in this period who were working on their thesis projects, collecting relevant patient data, reviewing pt charts, worked in a government-funded community hospital , a multi specialty hospital , and I am presently working on two clinical research projects related to trauma at XXXX Medical College/XXXX Medical Centre.
I have had an excellent opportunity to demonstrate my command of the clinical knowledge base and have been exposed to an extremely broad variety of cases ring my time of service at XXXX hospital, one of the biggest in South India. I am especially proud of my high level of dissection skills combined with my extremely high motivation for practicing surgery. My clinical rotations, in particular, have brought surgery home to me as much more than an art form, as also an act of love and solidarity that I find most gratifying. As someone who loves the arts, I greatly admire the way that surgery balances art and science almost as in a dance, requiring the highest levels of concentration and dexterity. I see difficulties as challenges, and this draws me to surgery in particular. Surgery is my stage, and becoming an ever-more skilled and practiced surgeon is the life course that is the fullest expression of my spirituality as well as artistry. I have a sharp, well trained eye and a quick mind, born from a long love affair with art, puzzles, and a broad spectrum of intellectual challenges. I set high standards for myself and I fervently believe that the capacity to be critical of one』s own work is central to the fullest level of success possible in surgery. In addition to possessing the qualities required of an aspiring surgeon, I believe that I also have the fortitude that accompanies having passed through many trials through which I have matured emotionally and professionally into someone who is highly adept at operating at optimal level under moments of enormously high pressure.
I seek a position with a surgery residency program to have the opportunity to further cultivate my maximum potential, a program such as yours with the structure and discipline required for advancement. I would be pleased to participate in research as a component of the residency program. My primary interest, however, is clinical training and an opportunity to struggle towards an ever fuller appreciation of the science of surgery. I spend my spare time outdoors and exercise rejuvenates me for a return to my rotations. I have worked as a volunteer with a community health centre, and thus better appreciate the way that health-care is rooted in the community.
